Your First Visit
We try to gather as much information from you and your dentist about what treatment you may need prior to your visit so that we can schedule the proper amount of treatment time. Occasionally we may need to schedule separate exam appointments to allow us to assess what treatment is required.
If we have scheduled you for treatment on the first visit (based on information obtained from your General Dentist), we will schedule a 2-hour block of time exclusively for you (we do not double-book). If, for any reason, you need to reschedule your appointment, we appreciate your consideration in providing us a minimum 24-hour notice so that we may give that time to another patient.
Please assist us in providing the following information at your first appointment:
- Referral slip from your dentist
- List of medications you are taking
- Insurance card or other insurance information
A parent or legal guardian MUST accompany all patients under the age of 18 on the initial visit.
Please ALERT THE OFFICE IF YOU HAVE A MEDICAL CONDITION such as diabetes, high blood pressure, artificial heart valves and joints, etc. Please let us know if you have a condition which requires that you pre-medicate with antibiotics prior to treatment (and please take pre-medication one-hour prior to your appointment).
Your dentist may email or send over a radiograph or digital image. We will also take our own images as part of your treatment record. There is no additional charge for imaging taken during the course of treatment. We use the latest and safest digital radiography.
